MoveWell Mobility partners with physicians to provide thorough mobility assessments and documentation supporting medically necessary equipment recommendations.
Referral Process
- Referral sent to referrals@movewellmobility.net at least 24 hours in advance of the estimated in home visit.
- Include PT/OT order/insurance information with card/demographic/medical history
- Include in your referral email an estimated date of service or time request for faster scheduling.
- A team member will then coordinate scheduling with you via email (most common) or by phone.
- One of our PT/OT clinicians will be ready to perform the evaluation after the referral is submitted and scheduling is completed.

A comprehensive assessment performed by a licensed therapist to determine the most appropriate mobility equipment based on medical needs, function, environment, and goals.
Coverage varies by insurance provider. We assist with obtaining information regarding coverage and authorization requirements.
Accepted insurances: Medicare, Cigna, Aetna, Humana, UHC, and BCBS.
UHC/BCBS we are out of network but we still take these insurances.
Other insurances not listed will need to be verified for any out of network benefits. HMO plans are also accepted.
We due charge for Co pays and they are expected to be paid at the time of evaluation.
We are able to accept patients under home health care provided they pay a $125 self pay rate.
We do accept Medicaid patients at a self pay rate of $125 along with completion of signed waiver. The signed waiver will be provided upon scheduling.
